Day 1: May 22nd, 2023

Arrive at LAX by 8:30PM on 5/22.

 

Depart LAX for flight at 00:15 on 5/23.

All team members must arrive at the airport 3.5 hours before departure for luggage check-in, TSA, and Customs as a group (for group check-in only).

 

Day 2: May 23rd, 2023

In the air.

 

Day 3: May 24th, 2023

Arrive CGK at 13:20.

Day 4: May 25th, 2023

Preparing Mission in Jakarta.

Mission in Jakarta.

Pricing

 

Total price of the trip is $3,500.00* - *All-inclusive - flight, lodging, & meals. Pricing excludes personal travel insurance & malpractice insurance costs. Pricing is subject to change (depending on the final number of participants and airline negotiations). Remaining amount from overpayment will be refunded.

 

A $1,000.00 non-refundable deposit is required by March 10th to confirm your participation. Final installment will be due by March 31st.

Cash or check is preferred: Please make checks payable to the "Wealth by Health Foundation" with the memo as "2023 Indonesia Medical Mission" sent to PO Box 27685, Los Angeles, CA 90027 or to our physical office at 3059 Del Mar Ave, Rosemead, CA 91770.

Alternative Payment Methods: 

- Zelle: Zelle account email is donations@wealthbyhealth.org. Please include in the Zelle memo the following- "Your Full Name  - 2023 Indonesia".

- Credit Card: If you decide to pay by credit card, please note that there will be a 3% payment processing fee. As a volunteer run non-profit we don’t make any profit and are only charging actual costs for this trip. The payment processing fee is what our organization incurs when credit card transactions are made. Link to credit card payment >>

Flight & Transportation

 

Participants are highly encouraged to join our group flight to maximize efficiency (taking less time to locate each other), for better pricing through airline negotiations, and to increase luggage space for medical equipment. Those on our group flight will also have transportation arranged to the local site.

 

However, we understand that you may have unique circumstances which require you to book your own flights. In that case, please keep in contact with us to ensure you are receiving the most up-to-date itinerary, as it may change.

 

Please note: If you are booking your own flight and arriving at a different time than our group flight, you will be responsible for arranging your own transportation to the local site on your own.

Insurance

​

For Healthcare Providers: Wealth by Health can assist with arranging overseas medical malpractice insurance, which has been quoted at $300. Please contact us if you would like us to purchase medical malpractice insurance for you. 

​

For All Participants: Travel insurance (including health coverage & travel cancellation due to any unforeseen circumstances beyond our control) is required. Travel insurance can be purchased though major travel insurance companies such as AIG Travel Guard (www.travelguard.com) . Participants can also reach out to another travel insurance agent of their choice.

Visas

 

Visa information will be submitted on your behalf by Wealth by Health. To obtain an entry permit for this trip to Indonesia, we need you to provide the following documents:

1.  A copy of your valid passport for a minimum of 12 months before expiration date.

2. A copy of your medical license (if participating as a healthcare provider).

3. A copy of your CDC COVID-19 Vaccination Card with up-to-date vaccinations (primary series + booster dose).

4. A visa photo (for submitting your visa application) - must be a color photo on a white background.

5.  If booking your own flight, please provide us with your flight details, including proof of return.  Otherwise, if joining us on the group flight, WBH will submit this document for you.  

6.  Evidence of ownership of travel insurance (including health coverage & travel cancellation due to any unforeseen circumstances beyond our control).
