Scholarship Terms & Conditions

  • To be awarded a scholarship, the recipient must enroll at an accredited college or university located in the United States. Scholarship proceeds will be remitted directly to the school that the recipient is enrolled and attending. The scholarship recipient may need to complete a Payment Form stating which school they will be attending, and specifically where at the school to send the award payment. If this form is not completed, the award will not be disbursed, and the recipient may forfeit the award.

  • Scholarship recipients will be required to submit a current photo and agree to grant the Wealth by Health Steps for Change Foundation permission to use their photo on any related media or publications.

  • By applying for and accepting a Scholarship you give consent for sharing and publication of your application details in the event of your application being successful.

  • In submitting an application and accepting the scholarship, you consent for your data to be recorded and used on the website or any other relevant media and publications relevant to our organization.

  • Scholarship awardment is conditional. Additional information (i.e. valid photo ID, social security number, a copy of parents’ tax return, college admissions letter, photo, final official transcript, a copy of your FAFSA, a copy of your full tuition statement from the University you will be attending, etc.) will be requested and verified if you are selected as a scholarship recipient.

    • A copy of parents' tax return: Scholarship recipient must provide us with a signed, full copy of both parents' 2023 tax return and/or a copy of both parents' 2023 IRS tax return transcript.

    • If your parents...

      • Are married, or are unmarried and live together​, and filed separately in 2023: Submit information for both 2023 tax returns.

      • Are divorced or separated: Provide information from your custodial parent's 2023 federal tax return. If your parents filed jointly for 2023, submit information regarding your custodial parent from that return. If your custodial parent has remarried, submit 2023 income tax return information for him/her and your stepparent. You will be advised if tax information from your noncustodial parent is required.

      • Are not legally required to file a federal income tax return for 2023 and did not file:  Submit a completed 2023–2024 Parent Non-Filing Statement form.

      • File a tax return in another country: Submit a 2023-2024 Parent Non-Filing Statement form indicating that they are filing a foreign return and attach translated documentation of their income, such as a foreign tax return, employer statement, etc. Convert all amounts to U.S. dollars. Please also submit a complete 2023-2024 Parent Income and Expense form.

  • Your information is safe with us. The Wealth by Health Steps for Change Foundation Privacy Policy applies to all Steps for Change Scholarships.

  • Scholarship winners will be contacted by email on the week of award winner announcement and may also be contacted by phone and mail.

  • In the event a scholarship recipient has a full ride to the college or university he/she will be attending or has secured sufficient scholarships from other sources, the scholarship award will be passed on to a reserve scholarship candidate selected by the Scholarship Committee.
     

  • The Wealth by Health Foundation reserves the right to withdraw a scholarship award at any time based on conditions set forth in these Terms and Conditions.

  • We reserve the right to update or change any of these Terms and Conditions and you should check this page periodically. These Terms and Conditions will remain in effect with respect to any changes in its provisions in the future, which will be in effect immediately after being posted on this page
